Some transgender and nonbinary people undergo phalloplasty and/or metoidioplasty as part of their medical transition process. Across surgical disciplines, a variety of resources are used to assist patients who are preparing for surgeries, including educational materials, workshops, peer support, and lifestyle changes. For gender-affirming surgeries, patients undergoing assessments to discern whether they are ready to undergo the surgery, and to assist them in achieving preparedness when needed. Little research investigates what resources are useful in helping patients to feel prepared to (...) undergo phalloplasty or metoidioplasty, and how assessments and resources can promote patient autonomy in the process. Respect for patient autonomy is one of the central tenets of ethical healthcare, yet historically, scholarship related to pre-surgical assessments for gender-affirming surgery has focused determining the ideal surgical candidate rather than respecting patient autonomy and ascertaining individual patient needs. This study sought to fill this gap by utilizing data from PROGRESS (Patient-Reported Outcomes of Genital Reconstruction and Experiences of Surgical Satisfaction), a cross-sectional, community-based survey of trans and nonbinary adults from the United States of America and Canada who had undergone one or more of these surgeries. Results revealed most participants (86%, n = 186) felt prepared to undergo surgery, though the majority of our sample (53%, n = 105) did not find referral letter assessments to be helpful. Peer support such as online resources/blogs were rated as highly useful, along with surgical consults. In a multivariable logistic regression, higher perceived preparedness was associated with identifying as queer (inclusive of gay, bi and pansexual compared to being straight), and feeling that one’s assessment process was useful (as opposed to not useful). Type of assessment was not significantly associated with preparedness; therefore, what is most useful when preparing for surgery may vary across individuals. Healthcare professionals who interact with preparing patients should develop new or utilize existing resources to assist patients in identifying their preparation needs and achieving preparedness. Our data supports assessments that center surgical care planning rather than assessing level of gender dysphoria. Future longitudinal research could further refine which assessment processes are most effective in helping patients who are preparing for these surgeries. Assessments should ensure that patients are appropriately prepared to undergo and recover from surgery through a robust process of informed consent. (shrink)

Harrowing stories reported in the media describe Covid‐19 ravaging through families. This essay reports professional experiences of this phenomenon, family clustering, as encountered during the pandemic's spread across Southern California. We identify three ethical challenges following from it: Family clustering impedes shared decision‐making by reducing available surrogate decision‐makers for incapacitated patients, increases the emotional burdens of surrogate decision‐makers, and exacerbates health disparities for and the suffering of people of color at increased likelihood of experiencing family clustering. We propose that, in (...) response to these challenges, efforts in advance care planning be expanded, emotional support offered to surrogates and family members be increased, more robust state guidance be issued on ethical decision‐making for unrepresented patients, ethics consultation be increased in the setting of conflict following from family clustering dynamics, and health care professionals pay more attention to systemic and personal racial biases and inequities that affect patient care and the surrogate experience. Me'ah She'arim includes 100 chapters dedicated to the subject of kibbud av va'em, the commandment to honor and respect one's parents. The author's style of writing is very elegant and flowery, rich in allusion to Biblical phraseology, Midrash and liturgy. The book has enjoyed, and is still enjoying, great acceptance. We respond to a Letter to the Editor regarding "Principlism and contemporary ethical considerations for providers of transgender health care." We address criticisms by Castle & Klein (2024) of blatant fatphobia related to the ethical elements concerning BMI restrictions for gender-affirming surgery. Our response corrects several mischaracterizations of the article and clarifies our position. My co-authors and I remain focused on advocating for patient-centered, ethically sound, evidence-based, and equitable healthcare policies.

The second part explores Lamming’s most famous work, In the Castle of My Skin (1953), which offers important tropes in Black existential thought that are synchronous with Frantz Fanon’s Peau noir, masques blancs (1952), but with a more detailed exploration of the concept of political complicity through Lamming’s portrait of the phenomenon of slime and (...) its correlate, the slimy individual. Elijah Hess has argued that, given the accuracy of Stalnaker-Lewis semantics, Molinists possess good reason to shift their position to neo-Molinism. Conceding the validity but denying the soundness of this argument, I contend that the Stalnaker-Lewis semantics is multiply flawed, especially in its definitions of □→ and ◇→. Based on corrected definitions of □→ and ◇→ consistent with Molina’s own thought, I show how Hess’s neo-Molinist square of opposition collapses and his neological stages of God’s knowledge are undermined, thereby (...) leading back to an original Molinism. The COVID-19 pandemic and its associated stressors have impacted the daily lives and sleeping patterns of many individuals, including university students. Dreams may provide insight into how the mind processes changing realities; dreams not only allow consolidation of new information, but may give the opportunity to creatively “play out” low-risk, hypothetical threat simulations. While there are studies that analyze dreams in high-stress situations, little is known of how the COVID-19 pandemic has impacted dreams of university students. The aim of this (...) study was to explore how the dream content of students was affected during the university COVID-19 lockdown period (March–July, 2020). Using online survey methods, we analyzed dream recall content (n= 71) using the Hall-Van de Castle dream coding system and Fisher's exact tests for sex comparisons. Preliminary results indicate that female students experienced more nightmares as compared to male students. Dream analysis found that, relative to normative American College Student (ACS) samples generated pre-COVID-19, women were more likely to experience aggressive interactions in their dream content, including increased physical aggression. Results indicate that university students did experience changes in dream content due to the pandemic lockdown period, with women disproportionally affected. These findings can aid universities in developing support programs for students by bringing forth an understanding of students' concerns and anxieties as they process the “new normal” of social distancing. _ Source: _Volume 54, Issue 2-3, pp 125 - 145 Maimonides’s _Guide for the Perplexed_ had a significant influence on both Jewish and Christian philosophy, although the vast majority of Jewish and Christian readers in the Middle Ages could not read the original Judeo-Arabic text. Instead, they had access to the text through Hebrew and Latin translations. The article focuses on words derived from the root _sh-h-r_ in the original text of Maimonides, first on the understanding of Maimonides himself, where (...) they take on two meanings; the first sense of these words is an adjective that refers to things well-known to the larger public; the second sense is that in which the opinions held by the public are opposed to the intelligibles. Second, while one of Maimonides’ Hebrew translators, Ibn Tibbon, did understand the original meaning of the words in the _Guide_, the other, Alharizi did not; he missed the distinction between rational understanding and generally admitted opinions. This misunderstanding changed the meaning of three important passages of the _Guide_. Finally the mistranslation of Alharizi influenced the medieval philosophers that either read his translation, such as Rabbi Aaron ben Elijah of Nicomedia, or a Latin translation based upon it, such as Meister Eckhart. Beer ’s paper devotes much energy to buttressing the walls of Castle Dynamic and dredging its moat in the face of what some of its dwellers perceive as a besieging army chanting “no cognition without representation”. The divide is real, as attested by the contrast between titles such as “Intelligence without representation” and “In defense of representation”, to pick just one example from each side. It is, however, not too late for people from both sides of the moat to (...) meet on the drawbridge and see if all that energy can be put to a better use. The parley can be organized around an attempt to identify those attributes of representations that both sides may find useful. In my view, one such attribute is the capacity for hierarchical abstraction, an aspect of the representational approach not actually mentioned by Beer. The capacity for abstraction is gained automatically by those theoretical frameworks that opt for the explanatory benefits of representation in understanding cognition. Moreover, without appropriately structured mediating states, a cognitive system would be incapable of dealing with complex reality. Thus, hierarchical abstraction is useful not just for the cognitive scientists in their attempts to scale up their understanding of cognition: it is also indispensable for cognitive systems that aspire to scale up their understanding of the world. Assuming this methodological perspective, it will attempt to interpret the “poetical mark” of the novel in order to mark it as the guide which leads our whole interpretation of the text, in which the distance and differences between K. and the villagers will be described. Finally, we will confront with R. Calasso’s description of the (...) class='Hi'>castle as a place of concretion of power according to our interpretation of the scheinbare Leere from the novel’s beginning and K.’s “title” of Landvermessen. Among the reasons that Whitehead is such an interesting philosopher is that his work resonates across philosophical traditions. This collection develops connections between Whiteheadian concepts and recent European thinkers. The purpose is not simply to compare, however, but, as editor Jeremy Fackenthal suggests, to develop a Whiteheadian thinking “in tandem” with European philosophers in order to create disruptions or “dislocations” in thought that can engender creative approaches to contemporary problems.One general feature of the book deserves mention at the outset, though (...) I will return to it later in the review: although phenomenology and hermeneutics are each represented in the volume, roughly half of the essays focus on the work of Gilles Deleuze (if we group these together with essays focused on “new materialism,” which is connected with Deleuze, the proportion is more like two-thirds). This will be no surprise, since Deleuze is the European philosopher who was most appreciative of Whitehead. And yet, the result is that the conversation with “continental philosophy” is highly selective.The book is organized into three parts, each comprised of three essays. In part 1, “Technological and Systematic Dislocations,” William S. Hamrick brings Whitehead into conversation with Maurice Merleau-Ponty's concept of “the flesh” to suggest an ecological ontology capable of overcoming the instrumentalization of nature. Hamrick's strategy is to read Whitehead and Merleau-Ponty as contributing to an “ontological foundation” for a sustainable civilization (18). Bo Eberle writes about the “quantified self” suggested in new, wearable self-monitoring technologies. Drawing on Whitehead's notion of causal efficacy, Eberle argues that devices such as Fitbit give us access to bodily states that bypasses the standard ways we are aware of ourselves (i.e., through consciousness). He goes further to argue that, like all ways of knowing, wearable technologies are selective, and that the selection reflects the interests of the manufacturers and promotes a certain kind of subjectivity (in the case of Fitbit, a subjectivity that is oriented toward “getting your steps in”). Whitehead's epistemology thus enables the critic of wearable technologies to understand how contemporary experience and self-understanding is shaped not simply by recent technological mediations but by the material interests embedded in them. J.R. Hustwit and Carl Dyke take on what they call “activist” uses of Whitehead that naively (in their judgment) suggest that a non-relational substance metaphysics is to blame for most of our social ills. They employ the theory of complex adaptive systems in conversation with Whitehead and Antonio Gramsci to show that, in fact, achieving significant social change is far more difficult and even unpredictable than mere insertion of better ideas into public discourse. Dominant hegemonies have ways of adapting and absorbing attempts to undermine them. Here Hustwit and Dyke draw on the philosophy of Paul Ricoeur to suggest that resistance to the dominant hegemony is nevertheless always possible because of the vagueness of discourse, allowing a plurality of competing interpretations and thus fractures in the dominant hegemony. However, even this kind of resistance is impossible to control and its effects incapable of being entirely predicted. To the essay's titular question, “Can Whitehead save the world?” the authors answer, “No.”In part 2, “Animal/Human Dislocations,” Jeremy Fackenthal draws on the new materialism of Rosi Braidotti (and Deleuze) along with Whitehead to argue for a “process-relational” nomadism that culminates in a “becoming-imperceptible.” His reading together of Whitehead and Braidotti creates the opportunity to recast the former's concept of objective immortality in such a way that it does not refer to death so much as to being diffused into a relational network. The satisfaction of an actual occasion may be taken as its translation into its effects among its relations. Fackenthal argues that this becoming-imperceptible makes possible a sustainable presence within a fragile network of life. Deena M. Lin uses Whitehead, Braidotti, and Judith Butler to lay out a vision of human life as precarious and deserving attention, care, and solidarity. She presses this vision as a direct moral assault on US (Trump's) policy toward Syrian refugees. Going further, she draws on Braidotti's posthuman account of zoe to argue for universal connection and solidarity with all life that should foster, as the title of her essay puts it, “Welcoming of Syrian Life.” Tano Posteraro offers an intriguing essay drawing on Jakob von Uexküll's account of animal experience. Posteraro argues that Uexküll's notion of an animal's Umwelt, shaped by its distinctive perceptual apparatus, needs to be supplemented by Whitehead's notion of prehension to account for the active dimension of animal perception. Rather than a passive registering of animal environments, Posteraro argues that the latter are constructed, and that concepts (or conceptual prehensions) should be understood as tools with which such environments are constructed.In part 3, “Time, the World, and Abstraction,” three essays probe ways that Whiteheadian and Deleuzian themes may resonate critically and constructively. Kris Klotz brings the thought of Whitehead and Deleuze together to inquire about how alternatives to present social forms are envisioned. He argues that, for Whitehead, every actual occasion involves a selection of some features of the past, and, further, that every selection is also an abstraction from the concrete welter of influences impinging on it. For Whitehead, then, to envision alternatives involves a critique of abstractions, a reinstatement of the concrete plurality of possibilities. This, for Whitehead, is what rationality does: it works at restoring the concrete, and, with it, a series of alternatives that have been eclipsed. To this Whiteheadian analysis, Klotz adds Deleuze and Guattari's critique of “opinion.” For Deleuze and Guattari, opinion is not an idiosyncrasy but a consensus to which differences of experience are subjected. Philosophy, for Deleuze and Guattari, must always struggle against opinion in order to bring alternatives to light. These conceptions of reason/philosophy, Klotz argues, represent useful ways to critique inequalities and other social wrongs that are embedded in the social forms that are routinely taken for granted. Elijah Prewitt-Davis uses a scene from Buffy the Vampire Slayer, along with philosophical innovations of Foucault, Deleuze, and Whitehead, to offer a distinctive account of power. Prewitt-Davis calls attention to Deleuze's important book on Leibniz, The Fold, and to how Whitehead's concept of prehension lies at its center. The upshot is that power is not reduced to dominance but, instead, is at work wherever any occasion of experience persists amid the onslaught of the influence of other occasions. Power, as Deleuze wrote, is the capacity to be affected—and to decide how to constitute oneself anew. Keith Robinson offers a detailed account of Whitehead's epistemology along with a reading of Deleuze's analysis of temporality to suggest that there is a common trajectory that seeks to undermine what the title of the essay describes as the “Bifurcation of Nature” (i.e., the separation of natural phenomena and human experience) within the dominant Western philosophical tradition.There is much of value in the volume as a whole. Aside from staging what Spinoza might have called good encounters between Whitehead and a number of continental philosophers, enhancing the conceptual power of each, the contributors are able to make a case that these philosophical trajectories, when read together, have much to contribute to our understanding of and response to the pressing issues of the day. In addition, readers who are interested in the relationship between Whitehead and the new materialism will find this volume to be a helpful resource, stocked with fine examples of how to sharpen new materialist thinking with Whitehead and how to read contemporary contexts through these lenses. On the other hand, this book is not a handbook nor a comprehensive reference. The organizational logic behind its division into sections is murky and the amount of attention given to particular continental philosophers as opposed to others appears somewhat arbitrary: there are recent continental philosophers who get little to no attention at (Derrida, Malabou, Žižek, Agamben, Latour, Nancy, for example), several of whom might prove to be quite interesting to read “in tandem” with Whitehead. We generalize two well-known model-theoretic characterization theorems from propositional modal logic to first-order modal logic. We first study FML-definable frames and give a version of the Goldblatt–Thomason theorem for this logic. The advantage of this result, compared with the original Goldblatt–Thomason theorem, is that it does not need the condition of ultrafilter reflection and uses only closure under bounded morphic images, generated subframes and disjoint unions. We then investigate Lindström type theorems for first-order modal logic. An algebraic semantics, based on factor algebras, for one-way and two-way implicative verbs is proposed. Implicative verbs denote elements of filters or of ideals generated by identity functions in factor algebras. This semantics explains in particular the problem of implicational equivalence raised by two-way implicative verbs, and shows that the negation necessary to establish the implicativity of these verbs is the negation which preserves the presuppositions of sentences with implicative verbs. The theoretical framework adopted in the exact sciences, for constructing and testing deterministic theories on the one hand, and modelling and analysis of observed phenomena on the other, is often implicitly assumed to be that of structural stability. In view of recent developments in nonlinear dynamics, it is argued here that in general it may not be possible to assume strict determinism and structural stability simultaneously; either strict determinism holds, in which case the fragility framework may turn out to be (...) the appropriate framework for the study of certain phenomena in the exact sciences, or ‘structural stability’ is restored at the expense of introducing stochasticity. The article studies two related issues. First, it introduces the notion of the contraposition of quantifiers which is a “dual” notion of symmetry and has similar relations to co-intersectivity as symmetry has to intersectivity. Second, it shows how symmetry and contraposition can be generalised to higher order type quantifiers, while preserving their relations with other notions from generalized quantifiers theory.
